Separate the VM page table backend operations from the state machine since the amdgpu_vm.c file is becoming to complex. The allocating, freeing and updating page tables and page directories can easily be moved into a separate file. While at it cleanup everything checkpatch.pl reported and rename the functions a bit to make more clear that they belong together.
