502ad6e5a6 x86/msr: Change the function type of native_read_msr_safe()
Modify the function type of native_read_msr_safe() to:

    int native_read_msr_safe(u32 msr, u64 *val)

This change makes the function return an error code instead of the
MSR value, aligning it with the type of native_write_msr_safe().
Consequently, their callers can check the results in the same way.

While at it, convert leftover MSR data type "unsigned int" to u32.

0c2678efed x86/pvops/msr: Refactor pv_cpu_ops.write_msr{,_safe}()
An MSR value is represented as a 64-bit unsigned integer, with existing
MSR instructions storing it in EDX:EAX as two 32-bit segments.

The new immediate form MSR instructions, however, utilize a 64-bit
general-purpose register to store the MSR value.  To unify the usage of
all MSR instructions, let the default MSR access APIs accept an MSR
value as a single 64-bit argument instead of two 32-bit segments.

The dual 32-bit APIs are still available as convenient wrappers over the
APIs that handle an MSR value as a single 64-bit argument.

The following illustrates the updated derivation of the MSR write APIs:

paravirt_write_msr() invokes cpu.write_msr(msr, val), an indirect layer
of pv_ops MSR write call:

    If on Xen:

            cpu.write_msr = xen_write_msr

Therefore, refactor pv_cpu_ops.write_msr{_safe}() to accept an MSR value
in a single u64 argument, replacing the current dual u32 arguments.

No functional change intended.

ed56a309f7 x86/msr: Add the native_rdmsrq() helper
__rdmsr() is the lowest-level primitive MSR read API, implemented in
assembly code and returning an MSR value in a u64 integer, on top of
which a convenience wrapper native_rdmsr() is defined to return an MSR
value in two u32 integers.  For some reason, native_rdmsrq() is not
defined and __rdmsr() is directly used when it needs to return an MSR
value in a u64 integer.

Add the native_rdmsrq() helper, which is simply an alias of __rdmsr(),
to make native_rdmsr() and native_rdmsrq() a pair of MSR read APIs.

__wrmsr() is the lowest level MSR write API, with native_wrmsr()
and native_wrmsrq() serving as higher-level wrappers around it:

However, __wrmsr() continues to be utilized in various locations.

MSR APIs are designed for different scenarios, such as native or
pvops, with or without trace, and safe or non-safe.  Unfortunately,
the current MSR API names do not adequately reflect these factors,
making it challenging to select the most appropriate API for
various situations.

To pave the way for improving MSR API names, convert __wrmsr()
uses to native_wrmsr{,q}() to ensure consistent usage.  Later,
these APIs can be renamed to better reflect their implications,
such as native or pvops, with or without trace, and safe or
non-safe.

No functional change intended.

795ada5287 x86/msr: Convert the rdpmc() macro to an __always_inline function
Functions offer type safety and better readability compared to macros.
Additionally, always inline functions can match the performance of
macros.  Converting the rdpmc() macro into an always inline function
is simple and straightforward, so just make the change.

Moreover, the read result is now the returned value, further enhancing
readability.

efef7f184f x86/msr: Add explicit includes of <asm/msr.h>
For historic reasons there are some TSC-related functions in the
<asm/msr.h> header, even though there's an <asm/tsc.h> header.

To facilitate the relocation of rdtsc{,_ordered}() from <asm/msr.h>
to <asm/tsc.h> and to eventually eliminate the inclusion of
<asm/msr.h> in <asm/tsc.h>, add an explicit <asm/msr.h> dependency
to the source files that reference definitions from <asm/msr.h>.

with X86_FEATURE_WRMSRNS when using token pasting to generate references
to X86_FEATURE_WRMSRNS.  KVM heavily uses token pasting to generate KVM's
set of support feature bits, and adding WRMSRNS support in KVM will run
will run afoul of the opcode macro.

While the GCC and Clang compilers already define __ASSEMBLER__
automatically when compiling assembly code, __ASSEMBLY__ is a
macro that only gets defined by the Makefiles in the kernel.

This can be very confusing when switching between userspace
and kernelspace coding, or when dealing with UAPI headers that
rather should use __ASSEMBLER__ instead. So let's standardize on
the __ASSEMBLER__ macro that is provided by the compilers now.

This is mostly a mechanical patch (done with a simple "sed -i"
statement), with some manual tweaks in <asm/frame.h>, <asm/hw_irq.h>
and <asm/setup.h> that mentioned this macro in comments with some
missing underscores.

a4cb5ece14 x86/cpufeatures,opcode,msr: Add the WRMSRNS instruction support
WRMSRNS is an instruction that behaves exactly like WRMSR, with
the only difference being that it is not a serializing instruction
by default. Under certain conditions, WRMSRNS may replace WRMSR to
improve performance.

Add its CPU feature bit, opcode to the x86 opcode map, and an
always inline API __wrmsrns() to embed WRMSRNS into the code.

176db62257 x86/boot: Introduce helpers for MSR reads/writes
The current set of helpers used throughout the run-time kernel have
dependencies on code/facilities outside of the boot kernel, so there
are a number of call-sites throughout the boot kernel where inline
assembly is used instead. More will be added with subsequent patches
that add support for SEV-SNP, so take the opportunity to provide a basic
set of helpers that can be used by the boot kernel to reduce reliance on
inline assembly.

Use boot_* prefix so that it's clear these are helpers specific to the
boot kernel to avoid any confusion with the various other MSR read/write
helpers.

46d28947d9 x86/extable: Rework the exception table mechanics
The exception table entries contain the instruction address, the fixup
address and the handler address. All addresses are relative. Storing the
handler address has a few downsides:

 1) Most handlers need to be exported

 2) Handlers can be defined everywhere and there is no overview about the
    handler types

 3) MCE needs to check the handler type to decide whether an in kernel #MC
    can be recovered. The functionality of the handler itself is not in any
    way special, but for these checks there need to be separate functions
    which in the worst case have to be exported.

    Some of these 'recoverable' exception fixups are pretty obscure and
    just reuse some other handler to spare code. That obfuscates e.g. the
    #MC safe copy functions. Cleaning that up would require more handlers
    and exports

Rework the exception fixup mechanics by storing a fixup type number instead
of the handler address and invoke the proper handler for each fixup
type. Also teach the extable sort to leave the type field alone.

This makes most handlers static except for special cases like the MCE
MSR fixup and the BPF fixup. This allows to add more types for cleaning up
the obscure places without adding more handler code and exports.

There is a marginal code size reduction for a production config and it
removes _eight_ exported symbols.

fdb46faeab x86: Use tracepoint_enabled() for msr tracepoints instead of open coding it
7f47d8cc03 ("x86, tracing, perf: Add trace point for MSR accesses") added
tracing of msr read and write, but because of complexity in having
tracepoints in headers, and even more so for a core header like msr.h, not
to mention the bloat a tracepoint adds to inline functions, a helper
function is needed to be called from the header.

Use the new tracepoint_enabled() macro in tracepoint-defs.h to test if the
tracepoint is active before calling the helper function, instead of open
coding the same logic, which requires knowing the internals of a tracepoint.

be261ffce6 x86: Remove X86_FEATURE_MFENCE_RDTSC
AMD and Intel both have serializing lfence (X86_FEATURE_LFENCE_RDTSC).
They've both had it for a long time, and AMD has had it enabled in Linux
since Spectre v1 was announced.

Back then, there was a proposal to remove the serializing mfence feature
bit (X86_FEATURE_MFENCE_RDTSC), since both AMD and Intel have
serializing lfence.  At the time, it was (ahem) speculated that some
hypervisors might not yet support its removal, so it remained for the
time being.

Now a year-and-a-half later, it should be safe to remove.

I asked Andrew Cooper about whether it's still needed:

  So if you're virtualised, you've got no choice in the matter.  lfence
  is either dispatch-serialising or not on AMD, and you won't be able to
  change it.

  Furthermore, you can't accurately tell what state the bit is in, because
  the MSR might not be virtualised at all, or may not reflect the true
  state in hardware.  Worse still, attempting to set the bit may not be
  successful even if there isn't a fault for doing so.

  Xen sets the DE_CFG bit unconditionally, as does Linux by the looks of
  things (see MSR_F10H_DECFG_LFENCE_SERIALIZE_BIT).  ISTR other hypervisor
  vendors saying the same, but I don't have any information to hand.

  If you are running under a hypervisor which has been updated, then
  lfence will almost certainly be dispatch-serialising in practice, and
  you'll almost certainly see the bit already set in DE_CFG.  If you're
  running under a hypervisor which hasn't been patched since Spectre,
  you've already lost in many more ways.

  I'd argue that X86_FEATURE_MFENCE_RDTSC is not worth keeping.

So remove it.  This will reduce some code rot, and also make it easier
to hook barrier_nospec() up to a cmdline disable for performance
raisins, without having to need an alternative_3() macro.

093ae8f9a8 x86/TSC: Use RDTSCP
Currently, the kernel uses

  [LM]FENCE; RDTSC

in the timekeeping code, to guarantee monotonicity of time where the
*FENCE is selected based on vendor.

Replace that sequence with RDTSCP which is faster or on-par and gives
the same guarantees.

A microbenchmark on Intel shows that the change is on-par.

On AMD, the change is either on-par with the current LFENCE-prefixed
RDTSC or slightly better with RDTSCP.

The comparison is done with the LFENCE-prefixed RDTSC (and not with the
MFENCE-prefixed one, as one would normally expect) because all modern
AMD families make LFENCE serializing and thus avoid the heavy MFENCE by
effectively enabling X86_FEATURE_LFENCE_RDTSC.

b3d7ad85b8 x86: Introduce barrier_nospec
Rename the open coded form of this instruction sequence from
rdtsc_ordered() into a generic barrier primitive, barrier_nospec().

One of the mitigations for Spectre variant1 vulnerabilities is to fence
speculative execution after successfully validating a bounds check. I.e.
force the result of a bounds check to resolve in the instruction pipeline
to ensure speculative execution honors that result before potentially
operating on out-of-bounds data.

No functional changes.

b24413180f License cleanup: add SPDX GPL-2.0 license identifier to files with no license
Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.

By default all files without license information are under the default
license of the kernel, which is GPL version 2.

Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier.  The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.

This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.

How this work was done:

Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
 - file had no licensing information it it.
 - file was a */uapi/* one with no licensing information in it,
 - file was a */uapi/* one with existing licensing information,

Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.

The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne.  Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.

The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed.  Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.

Criteria used to select files for SPDX license identifier tagging was:
 - Files considered eligible had to be source code files.
 - Make and config files were included as candidates if they contained >5
   lines of source
 - File already had some variant of a license header in it (even if <5
   lines).

All documentation files were explicitly excluded.

The following heuristics were used to determine which SPDX license
identifiers to apply.

 - when both scanners couldn't find any license traces, file was
   considered to have no license information in it, and the top level
   COPYING file license applied.

   For non */uapi/* files that summary was:

   and that resulted in the third patch in this series.

 - when the two scanners agreed on the detected license(s), that became
   the concluded license(s).

 - when there was disagreement between the two scanners (one detected a
   license but the other didn't, or they both detected different
   licenses) a manual inspection of the file occurred.

 - In most cases a manual inspection of the information in the file
   resulted in a clear resolution of the license that should apply (and
   which scanner probably needed to revisit its heuristics).

 - When it was not immediately clear, the license identifier was
   confirmed with lawyers working with the Linux Foundation.

 - If there was any question as to the appropriate license identifier,
   the file was flagged for further research and to be revisited later
   in time.

In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.

Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights.  The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.

Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.

In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.

Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
 - a full scancode scan run, collecting the matched texts, detected
   license ids and scores
 - reviewing anything where there was a license detected (about 500+
   files) to ensure that the applied SPDX license was correct
 - reviewing anything where there was no detection but the patch license
   was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
   SPDX license was correct

This produced a worksheet with 20 files needing minor correction.  This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.

These .csv files were then reviewed by Greg.  Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected.  This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.)  Finally Greg ran the script using the .csv files to
generate the patches.

a585df8eda x86/MSR: Carve out bare minimum accessors
Add __rdmsr() and __wrmsr() which *only* read and write an MSR with
exception handling. Those are going to be used in early code, like the
microcode loader, which cannot stomach tracing code piggybacking on the
MSR operation.

While at it, get rid of __native_write_msr_notrace().
