0c3beacf68 asm-generic: introduce text-patching.h
Several architectures support text patching, but they name the header
files that declare patching functions differently.

Make all such headers consistently named text-patching.h and add an empty
header in asm-generic for architectures that do not support text patching.

e081c14744 powerpc/e500: remove enc and ind fields from struct mmu_psize_def
enc field is hidden behind BOOK3E_PAGESZ_XX macros, and when you look
closer you realise that this field is nothing else than the value of shift
minus ten.

So remove enc field and calculate tsize from shift field.

Also remove inc field which is unused.

map_kernel_page() and unmap_kernel_page() have the same prototypes
on nohash/32 and nohash/64, keep only one declaration.

9864816180 powerpc/book3e: Fix PUD allocation size in map_kernel_page()
Commit 2fb4706057 ("powerpc: add support for folded p4d page tables")
erroneously changed PUD setup to a mix of PMD and PUD. Fix it.

While at it, use PTE_TABLE_SIZE instead of PAGE_SIZE for PTE tables
in order to avoid any confusion.
